Side effects, contraindications and regulatory status
Reported side effects
- Label statement (Novarel, urinary-derived hCG): hCG has not been demonstrated effective as adjunctive therapy in obesity; no substantial evidence it increases weight loss beyond caloric restriction; no known effect on fat mobilization or appetite.[1] A Approved label
- Novarel label, Warnings (use with menotropins): ovarian hyperstimulation, enlargement or rupture of ovarian cysts with hemoperitoneum, multiple births and arterial thromboembolism are listed as the principal serious adverse reactions.[1] A Approved label
- Pregnyl label, Warnings: ovarian hyperstimulation syndrome can progress rapidly and severe cases may be life-threatening; assess women for OHSS for at least two weeks after hCG administration; anaphylaxis reported with urinary-derived hCG.[2] A Approved label
- Novarel label, Adverse Reactions: headache, irritability, restlessness, depression, fatigue, edema, precocious puberty, gynecomastia, injection site pain; localized and systemic hypersensitivity reactions have been reported.[1] A Approved label
- Novarel label: sporadic reports of testicular tumors in healthy young men receiving hCG for secondary infertility; a causative relationship has not been established. Diluent benzyl alcohol is linked to fatal gasping syndrome in premature infants.[1] A Approved label
Contraindications
- Novarel label: precocious puberty, prostatic carcinoma or other androgen-dependent neoplasm, prior allergic reaction to HCG; the label also states HCG may cause fetal harm when given to a pregnant woman.[1] A Approved label
- Pregnyl label: prior hypersensitivity to human gonadotropins or excipients; high serum FSH (primary gonadal failure in women); uncontrolled non-gonadal endocrinopathies; abnormal vaginal bleeding of undetermined origin.[2] A Approved label
- Pregnyl label: tumors of the hypothalamus or pituitary gland, and of ovary, breast or uterus in females and breast or prostate in males; malformations of reproductive organs or uterine fibroids incompatible with pregnancy.[2] A Approved label
Regulatory status
US labels (DailyMed) for hCG products Novarel (Initial U.S. Approval 1974) and Pregnyl: Rx gonadotropin; label says not shown effective for obesity. Checked 2026-10-20.[1] Source

Summaries of the cited studies, each with its evidence grade.
dailymed.nlm.nih.gov[1]
A Approved label human
Novarel (urinary-derived hCG, Ferring) US label (DailyMed), intramuscular use only; label product, not a research peptide. Hypogonadotropic hypogonadism in males: 500 to 1,000 USP Units three times a week for three weeks, then the same dose twice a week for three weeks; or 4,000 USP Units three times weekly for 6 to 9 months, then may be reduced to 2,000 USP Units three times weekly for 3 months. Ovulation induction after menotropins: 5,000 to 10,000 USP Units one day after the last dose.
dailymed.nlm.nih.gov[2]
A Approved label human
Pregnyl (urinary-derived hCG, Organon) US label (DailyMed), intramuscular use only; label product, not a research peptide. Same regimens as the Novarel label: hypogonadotropic hypogonadism in males 500 to 1000 USP units 3 times a week for 3 weeks, then twice a week for 3 weeks; ovulation induction 5000 to 10,000 USP units one day after the last gonadotropin dose.
Ovidrel (choriogonadotropin alfa, recombinant hCG, EMD Serono) US label (DailyMed)[3]
A Approved label human
Ovidrel (choriogonadotropin alfa, recombinant hCG, EMD Serono) US label (DailyMed): a different product from urinary hCG; 250 mcg subcutaneously one day after the last dose of the follicle stimulating agent in ART or ovulation induction. Cited for identity only; no dose figure is attributed to hCG.
J Clin Endocrinol Metab 2005 (doi 10.1210/jc.2004-0802), randomized[4]
B Human trial human
J Clin Endocrinol Metab 2005 (doi 10.1210/jc.2004-0802), randomized: men given testosterone enanthate 200 mg weekly plus hCG 0 (placebo), 125, 250 or 500 IU every other day for 3 weeks; intratesticular testosterone rose linearly with the hCG dose and stayed in the normal range at low doses
J Clin Endocrinol Metab 2010 (doi 10.1210/jc.2010-0360), randomized[5]
B Human trial human
J Clin Endocrinol Metab 2010 (doi 10.1210/jc.2010-0360), randomized: after gonadotropin suppression with acyline, men received hCG 0, 15, 60 or 125 IU subcutaneously every other day for 10 days; intratesticular testosterone rose dose-dependently, from 77 nmol/l (0 IU) to 923 nmol/l (125 IU)
